Insurance is available for
full-time employees effective the first of the month following their hire
date. GMED.offers a health plan that includes prescription drug
coverage and one eye exam per participant per year. Insurance deduction occur
each pay period.
To help offset additional costs related to vision coverage, fulltime
employees are eligible to purchase additional coverage for up to $130 per
DEPENDENT CARE AND MEDICAL SPENDING ACCOUNTS
Employees may set aside pre-tax dollars in a Dependent Care Spending
Account to be used for qualifying childcare expenses and/or day care for
dependent adults up to $5,000 per plan.
Full-time employees may set aside pre-tax dollars in a Medical Spending
Account to be used for a wide range of out-of-pocket health care expenses
up to $2500 per plan year.
EMPLOYEE MEDICAL CARE REIMBURSEMENT
Full and part-time employees and their immediate family members are
able to receive reimbursement for medical care provided by Galichia
Medical Group providers.
Full-time employees are provided term life insurance equal to one times their
base salary. This coverage is provided at no cost to the employee.
Supplemental term life insurance is also offered to full-time employees.
VOLUNTARY SHORT TERM AND/OR LONG TERM DISABILITY INSURANCE
Short term and /or long term disability insurance is offered to all
full-time employees the first of the month following hire date.
PAID TIME OFF (PTO)
All part-time and full-time employees will accrue time to be used for
vacation, holidays, and sick days. Accrual rates are dependent upon status
of position and length of employment.
A 401(k) plan is available for all employees at the beginning of the month after their hire date or 21st birthday, regardless of their employment status FT/PT or PRN. Under the 401(k) plan, employees can elect to contribute up to 75% of their annual salary, subject to IRS limitations.
